Submit a Claim Form – Instructions

To qualify for a payment from the Net Settlement Fund, you must submit a timely and valid Claim Form. The Claim Form must be postmarked or submitted online no later than October 16, 2026. You are required to submit genuine and sufficient documentation for all your transactions and holdings of Eagle Pharmaceuticals common stock set forth in this Claim Form.


How to File Online:

After clicking the button below, you will be asked to provide your contact information, detailed information about the publicly traded common stock of Eagle Pharmaceuticals, Inc. (“Eagle” or the “Company”) between August 9, 2022 and October 1, 2024, both dates inclusive (the “Settlement Class Period”) you transacted and any documentation you have to provide to support your claim. Please have all your documentation as described above ready, as your claim will not be saved if you have to come back and finish at a later time.

After submitting your completed claim online, you will receive an email with a confirmation code for your completed submission. Be sure to keep your confirmation email and code and refer back to them if you have any questions about your Claim Form.

How to File by Email or Mail:

If you wish to submit a Claim Form via standard mail, you may download a copy of the Claim Form here. You will need to provide all the information requested on the Claim Form, attach any supporting documentation, sign it, date it, and then mail it to the following addresses:

Remember, all Claim Forms must be submitted online or received by the Claims Administrator no later than October 16, 2026. Please keep copies of everything you submit, as your claim may be audited, and you could be asked to provide additional information to process your claim.
